Chain Slipping..any ideas?

Since about a week ago..a couple of times on a ride,I get the chain slipping forward it seems,then the pedal catches as normal.
This only happened when I was on the smaller ring of 2 on the front mech and roughly the 4th/5th ring on the rear mech...I changed up/down and it doesn't happen again.
would this indicate a chain or a cassette issue..or maybe something else..any clues guys?

Tension in cable. Use barrel adjust to adjust accordingly

I had some problems with the tandem that sounded very like the above and decided, in my infinite wisdom, that it must be the freewheel (yes it is that old).
Changed freewheel and rear chain at grat expense - problem persisted.
On closer inspection I found that the teeth on middle and inner chainrings were worn down even beyond the "shark-tooth" stage.

So - look for worn teeth on the chain ring and the cassette first.

Experiencing this on my triple hybrid. Always around cog 5/6 and it's less than a year old with less than 500 miles.
Will try the barrel adjustment also
