Bow legged and pedal/cleat set up

I am bow legged, so much so that corrective surgery has been suggested, however even though I have arthritis in both knees unless I do something high impact I don't suffer much pain, which lead me into cycling as a passtime, as I spend more time on the bike I have noticed I tend to push with the outside of my foot and it leads to discomfort on longer rides.
I currently use spd-sl pedals, could I alter these to combat that?
Would spd pedals work better because they are more central to the foot?
Does anyone have experience with shoe inserts?

Any advice or knowledge welcome

U would be best to go and see a specialist bike fitter given your condition.

Trying to save money with first baby's arrival due monday. I have a set of spd-s which came on a bike and its not exactly ruining my cycling just wondered if there would be an easy fix
